Your matter has been scheduled for an appointment with Family Court Services.

Family Court Services (FCS) assists parents in addressing child custody and parenting time matters. Depending on your case, you will participate in confidential mediation and/or Child Custody Recommending Counseling (CCRC).

Please complete this form as fully and accurately as possible. The information you provide will assist the Family Court Services Specialist in understanding your family circumstances and the custody and parenting time issues to be addressed during your appointment.

Confidential mediation is a confidential process. CCRC process and report is not confidential between the Family Court Services Specialist, the Court, the parties, and their attorneys, but remains confidential from the public.
